Office Break In / Access to Limited Client Information
OFFICE BREAK IN/ACCESS TO LIMITED CLIENT INFORMATION
Over the July 4th weekend, there was a break in at Gulf Bend's Place4 suite, located at 6502 Nursery Drive, Victoria, Texas. Cash from the cash box was missing and Barbara Bohman's (Place4 Counselor) office was broken into. A locked cabinet in the Counselor's office that contained folders with minimal client information (Counselor's handwritten notes of initial interview; drawings) was also broken into. The incident was reported to the police who conducted an investigation.
The information in the Counselor's office did not include any financial type information or demographics (e.g. social security numbers, date of births, addresses) and most were only labeled with first name of individual and initial of last name. As indicated above, the folders included handwritten notes of initial counseling sessions and drawings by individuals. The Counselor did go through the folders and reported that nothing was missing and that information did not appear to be compromised. However, we cannot assure you that information was not read/viewed.
Gulf Bend Center has reviewed this incident, and the Privacy Officer has met with the Counselor and Clinical Director about this incident. Information was behind triple locks. Gulf Bend Center will continue looking into this incident and will determine what security measure may need to be implemented based on the police report.
