Public Information Request
Under the Texas Public Information Act, formerly known as the Open Records Act, you have the right to access information regarding the affairs of government and the official acts of public officials and employees and we may not ask why you want the information. All government information is presumed to be available to the public. Certain exceptions may apply to the disclosure of the information. Governmental bodies, such as Gulf Bend Center, shall promptly release requested information that is not confidential by law or information for which an exception to disclosure has been sought.
All Texas Public Information Act requests must be made in writing. Gulf Bend Center charges fees that are consistent with the fees allowed by the Texas Public Information Act. Gulf Bend Center will provide a cost estimate to you if the anticipated charges related to the request exceed $40.00.

If you would like a copy of your medical records, please call Gulf Bend Center at 361.575.0611 or 800.421.8825.
To make a public information request, you may submit the Center’s form or submit a different written request of your choosing by mail, personal delivery, fax, or email info@gulfbend.org. To make a written request, please print the Gulf Bend Center Public Information Request Form using this link and include the information outlined in the form.
*NOTE: Certain exceptions to disclosure of public information exist under the Texas Public Information Act. If Gulf Bend Center believes that an exception to disclosure may exist regarding any requested information, it may seek an opinion from the Office of the Attorney General.
